Lynn Agency, Inc. is an approved provider of the Consumer Directed Personal Assistance Program (CDPAP) by the New York State Department of Health. CDPAP enables the consumer to recruit, interview, hire, train, supervise, and dismiss his or her assitant.
The Consumer’s responsibilities under CDPAP
(i) recruit, interview, hire, train, supervise and dismiss their personal assistant(s); (ii) hire as many personal assistants as required to accommodate their health care needs and lifestyle; (iii) develop a working relationship with their personal assistant; (iv) receive assistance from Lynn Agency, Inc. in recruiting and training personal assistants, service coverage, and personnel management; and (v) receive information on issues that impact individuals who receive home care.
Consumers must be eligible for New York State Medicaid and be currently receiving or approved to receive personal care service by the Department of Social Services. Furthermore, the Consumer must be medically stable and self-directing or have a legal guardian who can make decisions for them.
Lynn Agency, Inc. provides CDPAP services to consumers who reside in Queens, Brooklyn, Bronx, Manhattan, Suffolk County, & Nassau County.
The Consumer Directed Personal Assistance Program gives self-directing consumers freedom and peace of mind over their home care services. Consumers become more involved in their own plan of care. Consumers are also given the responsibility to coordinate and manage their services by allowing them to interview, hire, train supervise and schedule their employees.
If you meet the requirements but are not currently receiving Medicaid benefits, you should still contact us for assistance in applying. If you qualify for Medicaid, CDPAP is at no cost to you.
